About Us

Dr. Jacob Mohr is a 2004 Graduate of the Ohio State University College of Dentistry. He was awarded the Waterpik Technologies Prosthidontic Award for excellence in crown and bridge. After graduation, he worked as an associate until joining the National Health Service Corps. During his five years with the NHSC, he treated uninsured and low-income patients in Lima. He was selected to be an adjunct professor for the Arizona School of Oral health and taught students at the clinic about dentistry and public health. Dr. Mohr decided to open his own practice in Delphos in 2013. Dr. Mohr is a member of the American Dental Association, the Ohio Dental Association, and the Northwest Ohio Dental Society. He lives in Van Wert with his wife and two sons. He enjoys golfing and photography.
